We require the following forms of ID:
All identification documents must be ORIGINAL.
One of the documents presented must show the applicant signature.
The name on the applicant’s identification must be the same or evidence of a change of name that clearly shows the link between their birth name and current name must be shown.
The Road Traffic Regulations 2014 requires you to declare any permanent, long-term medical condition. If you have a medical condition that may impair your ability to control a heavy commercial vehicle you may be required to seek medical advice.
The Department of Transport requires that all tests are video recorded. You will be video/audio and GPS recorded during the assessment. You will need to sign the camera acknowledgement in front of a DOT Agent. The recording is viewed by DOT Authorised Officers.

Heavy commercial eyesight standards must be met to ensure that the applicant has adequate vision to allow them to drive safely. We will conduct the eye sight prior to your training. If you wear glasses please bring them with you. If you do not meet the eyesight requirements we advise that you seek medical advice.
Operator Training and Assessments Pty Ltd issues Statements of Attainments SOA to students that have been assessed as competent in accordance with the requirements of the Training Package.
Recognition of qualifications issued by other Registered Training Organisations is the acceptance by one RTO of a qualification or SOAs issued to a student by a different RTO. Before granting the student with CT or RPL towards their chosen UOC, evidence will be verified.
