When making a booking for your course or assessment we require

  • Your Full Name
  • WA Drivers Licence Details or Learners Permit Details if no WA Licence (you do not need a Learners Permit if you have a WA Licence)
  • Contact Number
  • Email Address
  • USI Number

Payment Details

* Booking Deposit:- A $500 deposit is required to secure your booking for any course or assessment.

  • Credit/Debit Card over the phone
  • Cash/Card at office
  • Direct Deposit on request

Note:- The remaining balance is due on the day of your course/assessment.

Proof of Identify

We require the following forms of ID:

  • Drivers Licence / Learner’s Permit
  • 2 other forms of ID (eg: Medicare Card, Credit Card, Health Care Card, High Risk Word Card, etc)

All identification documents must be ORIGINAL.

One of the documents presented must show the applicant signature.

The name on the applicant’s identification must be the same or evidence of a change of name that clearly shows the link between their birth name and current name must be shown.

Health and Medical Conditions

The Road Traffic Regulations 2014 requires you to declare any permanent, long-term medical condition.  If you have a medical condition that may impair your ability to control a heavy commercial vehicle you may be required to seek medical advice.

Video Recording

The Department of Transport requires that all tests are video recorded. You will be video/audio and GPS recorded during the assessment. You will need to sign the camera acknowledgement in front of a DOT Agent. The recording is viewed by DOT Authorised Officers.

Eyesight

Heavy commercial eyesight standards must be met to ensure that the applicant has adequate vision to allow them to drive safely. We will conduct the eye sight prior to your training. If you wear glasses please bring them with you. If you do not meet the eyesight requirements we advise that you seek medical advice.

Statement of Attainment

Operator Training and Assessments Pty Ltd issues Statements of Attainments SOA to students that have been assessed as competent in accordance with the requirements of the Training Package.

Recognition of qualifications

Recognition of qualifications issued by other Registered Training Organisations is the acceptance by one RTO of a qualification or SOAs issued to a student by a different RTO. Before granting the student with CT or RPL towards their chosen UOC, evidence will be verified.
